I woke up this morning with severe back pain , especially in the left shoulder blade and lower back . In addition I have nausea. Do you know what could it be?

It could be a coronary heart disease, angina or chest or an aortic dissecting aneurysm . I would go to the ER immediately to assess your heart and blood vessels.
